Carotid Angioplasty and Stenting Versus Carotid Endarterectomy for Treatment of Asymptomatic Carotid Stenosis: A Randomized Trial in a Community Hospital

Kenneth Ouriel, MD

Cleveland Clinic Foundation, 9500 Euclid Avenue, Vasc Surg Desk S-40, Cleveland, OH 44195 ourielk{at}ccf.org

The authors conducted a study to determine if carotid artery stenting (CAS) is equivalent to carotid endarterectomy (CEA) in reducing the risk of stroke in asymptomatic patients with carotid stenosis. Patients (n = 85) were randomized to CAS or CEA and followed for 48 months. Both procedures were welltolerated. No major complications occurred. Hospital stay was similar. Stenosis decreased to an average of 59b after CAS and the patency of the reconstructed artery remained satisfactory regardless of the technique. The authors concluded that an experienced cerebrovascular team may effectively treat CAS with results similar to those achieved with CEA

Key Words: Carotid stenosis • carotid stenting • carotid endarterectomy • asymptomnatic
